Lasting Mark Online Waiver

As a participant in the Lasting Mark Incentive Trip, I acknowledge and agree to the following:

1. Personal Responsibility / Alcohol Consumption – I understand that although this is a company-sponsored trip, I am responsible for my own personal conduct. I agree to conduct myself in a respectful manner at all times; and I will consume alcohol responsibly and in moderation.

2. Staying on Property – The resort offers activities and experiences for a safe, enjoyable and fulfilling experience. For my own safety and well-being, Lasting Mark STRONGLY encourages me to remain on the resort property throughout the duration of the trip. If I choose to leave the resort, I understand that I do so at my own discretion.

3. Assumption of Risk – I acknowledge that international travel involves certain inherent risks, including, but not limited to personal injury, illness, theft, accidents, and other unforeseen circumstances. These risks may be heightened if I choose to leave the resort or consume alcohol. I voluntarily accept and assume all such risks.

4. Medical Care and Emergencies – I acknowledge that Lasting Mark is not responsible for providing or covering the cost of any medical care, treatment or medications I may require during the trip. I hereby consent, give authorization to, and release from liability; trip leaders to secure emergency medical treatment in the event I am unable to, and I agree to be responsible for the costs thereof.

5. Expenses – I understand that any medical expenses, property loss, or other personal expenditures that result during or from this trip are to be borne by me.

RELEASE OF LIABILITY

In consideration of the opportunity afforded, with full knowledge and acceptance of the risks associated with this trip; and with full understanding of the above conditions and risks, I hereby release, indemnify and hold harmless Lasting Mark, its officers, directors, employees, affiliates, agents and representatives from any and all claims, liabilities, costs or damages arising out of or relating to:

a. My personal decisions and conduct during the trip.

b. My activities and conduct outside of the resort premises.

c. Injuries, accidents, or illness (including those related to alcohol consumption) to myself or others.

d. Any loss or damage to personal property caused by me
